We will send you the current price list upon your request via mail or email. Please note that we will provide a detailed estimate for any services not included in the price list.
For customers outside our delivery local area, we ship via UPS delivery service. Call us at 781-788-0078 or go to our Contact Us and submit the “Information Request” form to receive a UPS shipping label via fax or email.
It is crucial to provide shipping information and a tracking number for your case. That way we will schedule production upon arrival of your package to our facility. If the materials do not arrive within a giventime period, we can then track your case. Without your shipping and tracking information, CUSP will not be aware that you are shipping a case. We will follow the same protocol when shipping your case to you.
Payment is due to Cusp Dental Laboratory within NET 30 days from the date of the monthly statement. Finance charges of 1.5% per month are levied on past due balances. These charges must be paid on undisputed invoices, along with payment of the invoices themselves. The doctor will be notified that the unpaid balance will be charged to the credit card on file. If there is a specific credit card you prefer to use for payment, please contact us accordingly. Cusp Dental Laboratory reserves the right to refuse release of finished cases in accordance with its payment terms.
